To make a subject access request please email, phone or write to us.  Please provide as much detail as possible. Please advise whoever you speak to that you are making a subject access request. If you are writing to us please mark it for the attention of the Practice Manager.  You can ask someone else to submit a subject access request for you for example a solicitor or family member acting on your behalf. You may be asked to provide confirmation that you have asked them to do this.
